An accident is a terrible event... notice the location of the accident.
As daveuris notes, there is definite time-jumping going on. I think this also happens in the first limo scene and I believe the car crash symbolizes the pulling of the trigger of the gun and the explosion of the bullet ripping into her head (note all the smoke after the collision, similar to the muzzle flash and smoke). Also, Rita is confused and disoriented as she leaves the car/scene symbolizing, I think, the transition from reality into her hallucination. Evidence the driver pulling the gun, symbolic of her taking the gun, having the fear of doing it, and then BOOM.
A suicide is a terrible event.
The suicide takes place in the bedroom.
The suicide is linked to the car crash.
The bedroom is linked to Silencio.
The bedroom, Silencio and the car crash are all inextricably linked.
Does anything happen at Silencio that could be considered an accident in Lynch's terms???

ps. thanks to msdragon for the quote.

-I think that the symbolic "accident" is the fact that Betty got pregnant from Adam (it happens at Adam's address).
-And the litteral accident is the fact that the child gets killed in the second crash.

Jro made the connection of the street lamp we see through the first limo stop with both the microphone in Silencio and with the Dreamer.

So, the first limo stop was like a pov shot of RdelR's approach to the microphone. But the line was rushed, a thousand robins came with a beautiful memory.

Next time, using perfect timing, those responsible will first let "La Lloranda" sing her song for their love around the dinner table before ushering her up to the microphone (this time disguised as a bedpost) before showing her the mirror.

"Rita" carried within her two or three others. Rebekah also was the focal point of at least the two girls' full acquiesence, so she carried at least them within her also.

Jro made the connection of the streetlight through the limo window
with the Silencio microphone and with the Dreamer.

"Rita" in the limo, and Rebekah onstage, were both versions edit - rather doppleganger versions of the same concept
of the Three-in-one woman in the radiator From Eraserhead.

The streetlight/microphone-Dreamer, being a version of the real dreamer, was also a mirror (for the Dreamer to see herself through the dream eyes of Betty or Diane). This look into the mirror was what the hitmen were ushering "Rita" out of the limo for; they do not have the power to kill in the Dreamer what they wanted dead: only the Dreamer can do this.

The line was rushed by the hitmen. The robins came.

Actually, the rushing of the line may have been letting "Rita" swallow the Dreamer's identity too soon. Or both.

But next time they would make their three-in-one Dreamer sing her confession (as entertainment, at the dinner party), like Rebekah, before showing her the mirror.
